Tesamorelin is a synthetic analogue of growth hormone-releasing hormone (GHRH) consisting of the full 44-amino acid GHRH sequence with a trans-3-hexenoic acid group conjugated to the N-terminus. This modification enhances metabolic stability compared to native GHRH, extending its half-life and making it a more suitable research tool for studying GHRH receptor pharmacology and GH axis modulation.

Clinical Research Background

Tesamorelin is notable as one of the few GHRH analogues to have progressed through Phase 3 clinical trials, receiving FDA approval (as Egrifta) for HIV-associated lipodystrophy — specifically, the reduction of excess visceral adipose tissue in HIV-positive patients on antiretroviral therapy. This regulatory history provides a substantial clinical data package that researchers can draw upon when designing preclinical studies.

The clinical programme demonstrated dose-dependent reductions in visceral adipose tissue (VAT) measured by CT scan, along with improvements in trunk-to-limb fat ratio, without significant effects on glucose homeostasis at therapeutic doses. Mechanistically, tesamorelin stimulates pulsatile GH release from the anterior pituitary via GHRH receptor activation, with downstream IGF-1 elevation driving adipose tissue lipolysis.
